This is a problem I've had since I got GTA V for PC. I get a lot of stuttering in certain areas when I have the ingame v-sync option disabled. So usually I just tend to have it on instead, this allows me to play the game with any graphical settings I tried so far (which is mostly high or very high) and a smooth framerate without any noticable drops whatsoever. But since people keep saying that the ingame v-sync is pretty terrible and maybe even makes people slower in races I am now trying to limit the fps on through the graphics card's control panel. Enabling v-sync there and seperately also limiting the framerate to 60 in a profile dedicated to GTA (which is active) seems to make the fans go faster on my graphics card. And even with lowest ingame settings, the framerate still drops quite often (which also seems to lower usage of the CPU). I don't have a clue why this happens and am not a pro when it comes to PCs in general, any help is appreciated. Btw: Is the ingame v-sync really that bad? Because if not, using it would be the easiest solution. Just tried to change directX, then this happened. Graphics card: Radeon R9 200 Processor: i5 with 3492MHz, according to Steam RAM: 16GB GTA runs on a SSD.

I have done some more testing with different settings and ingame v-sync either on or off. Having it on always gives me a smooth experience, though it had an effect on the top speed of the RE-7B. Strangley enough having grass settings on ultra makes it go faster. (Speedometer in the car used to go to about 160, now it sometimes tops out)
When ingame v-sync is off (with the 60fps lock on the gpu side) the fans on my PC are louder, so it seems to put more stress on the system. I don't get a good framerate with any of the settings. On very high it's contant stuttering/fewer fps. This gets a bit better on lower settings, but then these strange fps-drops occure.

Generally it is a bit lower, this was just free roam at night, also my graphic settings are very high right now, as it seemed to make the cars a bit faster. After the editor update this doen't seem to be the case anymore, though. Right now I am using ingame v-sync again.

After the editor update this doen't seem to be the case anymore, though. Right now I am using ingame v-sync again. Use a program like MSI Afterburner's on screen display to check CPU usage in game. Often times CPU overload leads to frame drops. In this case you're a victim of Rockstar and we all suffer from this. Nothing you can do.
